Practitioner hasn’t taken into account why their habits have changed. We need to assess ‘why’ their habits have changed and not just fix effects, as the causation will likely reoccur and therefore fall back into their unhealthy habits.

Practitioner has assumed what they should do to help, by prescribing a set idea without consulting what the patient thinks and feels.

Practitioner hasn’t consulted with the patient what they currently portray as a healthy goal for themselves. Has prescribed again what they should do.

These are just some examples, consider some other notes you made that you feel would have made this conversation a ‘healthy conversation’
